Required Information When Reporting

Patient information
Full name, address, telephone number, date of birth, gender, and race/ethnicity.

Disease information
Disease being reported, date of onset, hospitalization status and date of admission, and pregnancy status and estimated due date.

Diagnostic information
Name, address, and telephone number of the diagnostic facility (where the case sought care), and the full name, address, and telephone number of the healthcare provider.

Reporter information
Full name of the person reporting the case, the name of the facility reporting, and the telephone number of the person or facility.

Laboratory information
Name of the laboratory performing the test, laboratory’s name for the test, specimen source, specimen collection date, testing results, test reference range, and test status (preliminary, final, amended and/or corrected).

How do you report? 

Disease Report Forms and relevant laboratory results should be faxed to 801-525-5210. Please include the required information when reporting.

Disease reporting via phone is available 24 hours a day, 7 days a week. Calls made after-hours or during weekends and holidays will be sent to a confidential voicemail system. This reporting line also allows for emergency calls to be forwarded to an on-call staff member. All messages will receive a prompt response.

All disease reports sent to Davis County Health Department are accessible only to certain staff to ensure patient confidentiality.

Who has to report?

Anyone with knowledge of a reportable disease is required to report to public health. However, most reports will come from healthcare providers, laboratories, healthcare facilities, schools, and childcare programs.
